An acknowledgment is a declaration by the officiant (the person signing the document) to a notary public, confirming that they have signed the document willingly. This process is common for various legal documents, including affidavits, and contracts.

The notary administers an oath or affirmation to the Affiant, which typically includes the phrase "sworn (or affirmed) to and subscribed before me" followed by the date and the notary's signature and seal. This process is common for affidavits and other documents where the signer is making a sworn statement.

1. Oath: If the affiant prefers or is required to swear an oath, the notary will have the affiant raise their right hand and ask them to repeat an oath such as "Do you solemnly swear that the statements in this document are true, so help you God?"
2. Affirmation: If the affiant prefers to affirm or if swearing an oath conflicts with their religious beliefs, the notary will ask them to make an affirmation, such as "Do you solemnly affirm that the statements in this document are true?"
